Mutimorbidity Integrating Care Study
The purpose of the MICS Study is to investigate whether integrating telehealth into Chronic Disease Care Plans (CDCPs) can improve outcomes for individuals with multimorbid chronic diseases.
Redefining integrating care to improve health outcomes for people with multimorbid chronic conditions in rural, remote and very remote Western Australia.
Study Settings
Recruiting 42 General Practices in rural and remote WA, with 6 General Practices in very remote WA.
Target Population
- Patients aged 18+ with multimorbidity
- Clinicians
With support from RAGCP Registrars.
